Migraine-Like Headache Attributed to Cerebral Cavernous Malformation

Abstract
Cerebral cavernous malformation (CCM) refers to intracranial vascular malformation. We report a patient with chief complaint of migraine-like headache probably attributed to CCM. A 51-year woman visited our outpatient headache clinic. The characteristics of her new-onset headache were as follows: episodic, lasting 2-24 hours, bilateral, pulsating, moderate-to-severe intensity, and nausea and/or vomiting. Headache attack always began after a strong perception of a foul smell, and occurred three times for 9 days. Brain MRI revealed early subacute hemorrhage within CCM associated with developmental venous anomaly in the left mesial temporal lobe. Surgical removal was performed to reduce the risk of recurrent hemorrhage. During 5-year follow-up, she has not experienced the headache attacks.
